E0445 is the HCPCS code for a standalone pulse oximeter — a device that measures peripheral oxygen saturation (SpO2) via a finger or ear probe. Unlike integrated oximeters built into CPAP machines or oxygen concentrators, E0445 is a standalone diagnostic device billed separately.

Standalone pulse oximeters have very limited DME coverage in Medicare. Most home oximetry is either bundled into other device payments or covered through different benefit categories (diagnostic tests vs. DME).

My patient uses CPAP but we want to monitor their oxygen saturation at night. Can I bill E0445?

Not typically. Most CPAP devices (E0601, E0470) include built-in oximetry, and the oximetry function is considered part of the device’s capability, not a separately billable item. Billing E0445 in addition to E0601/E0470 is likely to be denied as a duplicate or non-covered item. If the CPAP device does not include oximetry and the physician has documented a specific clinical need for standalone monitoring, E0445 may be justified.

Can I bill E0445 for a patient on oxygen therapy?

Unlikely. Oxygen equipment (E1390, E0431) either includes oximeters or can be paired with them. A standalone E0445 billed for an oxygen patient will likely be denied as unnecessary or bundled. The only exception would be a physician-documented specific clinical reason why the oxygen system’s oximetry is inadequate for this patient’s monitoring needs.

Is E0445 covered for home health monitoring of COPD patients?

Medicare Part B covers some home oximetry monitoring under specific home health scenarios, but it’s typically billed as a diagnostic test under Part B, not as DME under Part B E0445. If the patient is receiving home health services and the home health agency is billing oximetry, the supplier billing E0445 separately creates a conflict. Confirm the billing pathway with the home health agency before submitting.
