What this equipment is
E0562 is the HCPCS code for a heated water chamber humidifier that attaches to CPAP or BiPAP devices. The humidifier adds moisture to the pressurized air, reducing dryness, congestion, and nose/mouth irritation — the most common reason patients abandon CPAP therapy.
Heated humidification significantly improves CPAP adherence, particularly in dry climates and for patients with chronic sinus conditions. It's a separate billable item from the device itself.
Medicare coverage criteria
- Active covered CPAP/BiPAP device. Like A7030, E0562 requires a corresponding covered device (E0601 or E0470) on file. Humidifiers don't have their own separate medical necessity — they're covered as an adjunct to covered PAP therapy.
- Physician order. The ordering physician must specify heated humidifier on the order. Many physicians include it as a matter of course; others require clinical justification if the payer audits.
- Documentation of need. Ideally, physician notes documenting upper airway dryness, nasal congestion, or CPAP intolerance that humidification addresses. For payer audits, having this on file is critical.
What actually causes denials in DME back offices
- CO-97 (bundling) during the device rental period. Some payers bundle E0562 into the CPAP rental payment during the first 13 months, denying E0562 as CO-97 "service bundled." This is incorrect for Medicare — E0562 is separately billable — but it happens. The fix is an appeal with the LCD showing separate coverage.
- No device on file. Billing E0562 after the device rental has ended without updating the patient's device status in your billing system creates a same-or-similar or coverage gap issue.
- Humidifier billed as owned after 36-month cap. Wait — this is different from CPAP. The humidifier has different rules: if it was rented separately, the cap may differ. Verify rental month counts carefully when billing E0562 on a patient past the device rental cap.
- Missing KX modifier. Same as A7030 — the KX modifier certifies LCD criteria are met on Medicare claims.

- Patient-owned device record updated in billing system
- Confirmation device (E0601/E0470) is active or device ownership is recorded
Reimbursement note: Medicare allows approximately $60–$90 per E0562 depending on region. Commercial payer rates vary. Note: E0561 is a non-heated humidifier — a less commonly used code. Most patients are prescribed the heated version (E0562). Verify your payer contracts for both codes, as non-heated humidifiers sometimes have different reimbursement rates.

Is the heated humidifier bundled into the CPAP rental payment?
No for Medicare. E0562 is separately billable and separately reimbursable from E0601, even during the 13-month rental period. If you receive a CO-97 denial bundling E0562 into the device rental, appeal it — Medicare policy supports separate billing of E0562.
Can I bill E0562 for a patient who owns their CPAP device?
Yes. After the device rental cap, supplies and accessories (including E0562) continue to be covered. The patient’s ongoing medical necessity for PAP therapy is what drives coverage — device ownership doesn’t change the accessory coverage rules.
What’s the difference between E0561 and E0562?
E0561 = non-heated humidifier (passive water chamber). E0562 = heated humidifier (actively warms the water). Heated humidifiers are the clinical standard for CPAP adherence and the most commonly billed code. Most payers cover E0562; coverage for E0561 is less consistent.
